Lesson Request Forms:

Payment Note: If you are only booking group lessons please include your payment with your schedule or call the office with a credit card number.

If you are booking groups and private lessons which need to be confirmed, please pay for the lessons all together once the private lesson times are confirmed.

• Monthly Lesson Schedule PRINT Request form (February) (FOR PRINTING ONLY) (due between January 15 and 31).
• Monthly Lesson Schedule, MS Excel version (February)--(If you get a login prompt, just cancel and the file will open) Form can be filled in online and e-mailed in if you have Excel software.  Be sure to save a copy to your computer before e-mailing (and note where you've saved it).
